In order to apply for your Prime card reloadable upgrade, please print and complete the upgrade form which can be found here.

Along with the form, you will need to enclose a photocopy of one of the following pieces of identification:

Either: A photocopy of the photo page of your machine readable passport *
Or: A certified photocopy of the photo page of your current passport;
Or: A certified copy of your EU national issued ID card;
Or: A certified copy of your immigration status document, which shows both your residents permit and your photograph.

The certification must read as follows:

This is a true likeness to {applicant’s name}
Signature of the person certifying
Name of the person certifying and their position or capacity
Contact address of the person certifying
Contact number of the person certifying or name of branch if certified at the Post Office or company / institutional stamp Date of certification

Your document may be certified by any one of the following individuals:
Member of Parliament, Justice of the Peace, Minister of Religion, Doctor, Lawyer, established Civil Servant, Bank Officer or a person of similar standing.
